The Board of Supervisors is asked to approve a Master Agreement with Reach Out West End, a limited liability company, for the use of the Whole Person Health Score (WPHS) Assessment Tool. This tool, developed by Riverside University Health System, assesses a patient's overall well-being across six dimensions: physical health, emotional health, resource utilization, socioeconomics, ownership/activation, and nutrition/lifestyle. The assessment takes approximately 15 minutes and provides immediate results to aid care teams in making coordinated referrals to resources such as behavioral health services, veteran's benefits, and food assistance.

First 5 Riverside County will serve as the County Lead Agency for this agreement. Reach Out West End will integrate the WPHS into its client intake process, allowing them to assess clients' health status comprehensively and connect them to appropriate public programs. The agreement grants Reach Out West End limited access to County Information System data for client registration, authorization for data use, WPHS scores, referral types, referral confirmations, and closeouts. Participation is voluntary and requires client authorization. There is no cost associated with this agreement for the County.
